Items Needed for Registration

Certified copy of the child's birth certificate or other form of legal identification

Examples include:

  • Driver’s License

  • Passport

  • Military ID

  • Hospital birth record

  • Adoption records

  • Any other legal documentation that establishes identity of children

Child's Social Security Card

Required- Child's immunization record validated by a physician's signature or stamp, ImmTrac is acceptable

Appropriate Pre-K eligibility documentation (Pre-K students only)

Income Verification

  • Check Stub(s) Most Recent

  • Income Tax Return from prior year

Proof of Residency

A.  If living in own residence or renting

  • Provide current utility bill (water, gas, or electric) showing name, current address and date

  • Provide current rental agreement or lease (If renting or leasing)

B.  If building In Mercedes ISD

  • Provide builder's contract showing projected completion date (must be less than six months)

  • Campus may follow up (at projected completion date): provide utility bill (water, gas or electric), utility deposit, or connection confirmation showing new address

C.  If living in another person’s residence

  • Provide parent/guardian driver’s license or government issued photo ID

  • Provide driver’s license or government issued photo ID of the owner of the residence

  • Provide current utility bill (water, gas, or electric) showing name of owner, the current address and date

  • Resident owner MUST complete Proof of Residency that requires a notarized letter. This requires the person living with the owner to be present and sign the notarized letter as well.

  • If a child/children are not living with a parent, appropriate Court Documentation, State Agency Documentation....next school year- provide attending school with same/current information for proof of residency including Temporary Guardianship Letter signed by a parent/guardian and notary.

New to the District: If Living in another person’s residence

  • Go to Safe Schools/ Student Services Department for a clearance form

  • Provide current utility bill (water, gas, or electric) showing name of owner, the current address and date

  • A truancy officer will be sent to verify address provided. (If Needed)

  • Safe Schools will call parent/guardian to come in to get a clearance form. In addition, a notarized letter will be required.  Resident owner MUST complete Proof of Residency that requires a notarized letter.  This requires the person living with the owner to be present and sign the notarized letter as well. 

  • If a child/children are not living with a parent, appropriate Court Documentation, State Agency Documentation is needed.

All residency affidavits are subject to verification by campus staff/ truancy officer and if falsification of information is determined, withdrawal is immediate in accordance with FD (legal)
